 If your are a broker or policyholder who needs to
 report a claim, you have
several options:

  1. Send the incident report by e-mail to us at  ReportClaims@CertusClaims.com ;

  2. Go to our website http://www.certusclaims.com , complete the claim reporting form and e-mail it to us at the same e-mail address (Emily has updated our website);

  3. Fax the incident report to us at (805) 987-8806;

  4. Mail the report to us at P.O. Box 1030,
    Camarillo, CA  93011-1030; or,

  5. Call the report in to us at (805) 987-8803.
